Ring the practice on 0161 983 9500, send in a request here or book via NHS App

Contact reception and they will book a telephone appointment with the Nurse.  

Ideally 6 weeks prior to the patients travel plans.

The ‘Pre-Travel questionnaire’ is to be filled out and the patient needs to complete prior to the Nurse calling.

 The Nurse will call at that appointment date to discuss and review immunisation history and then the nurse will directly book the patient in for a face to face with herself if required.
